Year 9 DTP/MEN ACWY Vaccinations
Dear Parent,
DTP/MEN ACWY vaccinations will be taking place for Year 9 students in College on Thursday 8th February. Further information with the link to the consent form can be found in the attached letter.
It is really important that you complete the E-Consent form. Even if you decline the vaccine, Vaccination U.K. still need the consent form completed. This will ensure that your child does NOT get vaccinated.
If you are unable to access E-Consent, please contact Vaccination U.K. on: 01527 390030 and they will be able to assist you with this.
Please be aware that the deadline for the completion of this E-Consent is 31st January.
